Nursing Support Services are provided in accordance with the participant’s approved Person-Centered Plan (PCP) and any applicable authorization requirements established by the Developmental Disabilities Administration (DDA). Our team collaborates with all members of the participant’s support network to ensure continuity of care and the highest standard of service.

Our licensed Registered Nurses (RNs) provide comprehensive clinical oversight, health education, and ongoing support to help participants maintain optimal health while living as independently as possible.

Our Nursing Support Services Include:

• Comprehensive Nursing Assessments – Evaluating each participant’s health status and developing individualized nursing recommendations.

• Health Case Management – Coordinating healthcare services, monitoring medical needs, and communicating with physicians, specialists, and caregivers.

• Medication Management & Delegation – Providing medication oversight, delegation of nursing tasks where appropriate, and ensuring safe medication administration practices.

• Nursing Consultation – Offering professional guidance to participants, families, caregivers, and support staff regarding healthcare needs and best practices.

• Staff Training & Clinical Supervision – Educating and supervising direct support professionals to ensure they are equipped to provide safe, effective, and person-centered care.

• Health Education & Wellness Support – Promoting healthy lifestyles, disease prevention, and ongoing health monitoring to improve participants’ overall quality of life.
